Application
Form shall be used when submitting an application,
under the Electronic
Transactions Ordinance (Cap. 553) ("Ordinance"), to
the Government Chief Information Officer ("GCIO")
for recognition in any one or more of the following situations :
for recognition as a recognized certification authority ; or
for renewal of recognition as a recognized certification authority; and/or
for recognition of certificates.
The Notes
to Application Form provide information on the particulars and
documents that shall be furnished to the GCIO when the applicant
applies for recognition as stated above.

Electronic Submission
If any of the particulars and documents being
part of an application for recognition is submitted electronically, the submission shall follow the format, manner and procedure stipulated in the notice made under section 11(2) of the Ordinance on 9 August 2003 and published in the Gazette (G.N. 5651) on 22 August 2003, or any updated version of the same notice made and published under section 11(2) of the Ordinance from time to time. Where signature(s) are required on any of the particulars and documents that are submitted in electronic form, the particulars and documents concerned shall be digitally signed. Such digital signature(s) shall be supported by recognized certificate(s).
Application in respect of Qualified Person (i.e. An Assessor)
A CA shall apply in writing to the GCIO for approval that
the person whom the CA intends to engage for the preparation
of an assessment report is a qualified person under the Ordinance,
and furnish the GCIO with the
required documents and information in respect of the application.
