Written Requests for Information of OGCIO
Chief Executive Officer (Administration) (CEO(A)) is designated as the Access to Information Officer of the OGCIO who is responsible for promoting and overseeing the application of the Code.
Written requests for information held by the OGCIO should be addressed to :

Requests may be made by letter or by application form.
The application form is available at the following Reception Counters of the OGCIO or at all District Offices :
o 15/F, Wanchai Tower, 12 Harbour Road;
o 19/F Wu Chung House, 213 Queen's Road East;
o 6/F North Point Government Offices, 333 Java Road; or
o 6/F Cyberport 1, 100 Cyberport Road.
Alternatively, you may fill in the electronic application form or downloading the form (in PDF).
The following photocopying charge may be levied in relation to your application :

HK$1.0 per photocopy unless otherwise provided by enactment or approved by the Secretary for the Treasury
The above charge is subject to revision from time to time.
The following records are also available for public inspection at the office's Reception Counters at Wanchai Tower, Wu Chung House, North Point Government Offices and Cyberport 1 :
o organisation charts of the OGCIO;
o information on the services provided by the OGCIO;
o a list of the OGCIO's own records by category;
o a list of information either published or otherwise made available, whether free or on payment;
o an information sheet on the procedures and charges for access to information not routinely published; and
o the Code and the Guidelines on Interpretation and Application.
